I'd as soon it be the latter; there's plenty of room for drama without killing off a child. You could, for instance, make her spend a long time recovering and thus hide Derpy's sunshine, so to speak, behind a cloud of worry.
Point taken; she could, for instance, spend most of her foal's fillyhood in hospital hallways not listening to ponies who tell her that she did everything equinely possible. That is well within her fanon characterisation.
*shudders* that song... even when I was a child it creeped me out, when I was older and heard the whole thing...not just the chorus they teach kids, I understood why. Sunshine is a depressing song... and this scene looks heart-wrenching.
